Life at Pacific Heights Transitional Care

You Matter to Us

Enjoy Excellent 24-Hour Care

care

We cherish every patient who walks through our doors, and we are determined to help him or her succeed. We accomplish this by developing a culture of connectedness among our staff and taking a personal approach with each resident to ensure exceptional care is considered in every detail.

When new patients are admitted into our facility, they and their family members meet with a representative to discuss questions, review the individualized treatment plan, and discuss expectations. Our vision is to improve the health and well-being of our patients through quality care, and we aspire to do that every day.

Our highly trained staff of registered nurses, licensed vocational nurses, and certified nursing assistants are here 24 hours a day, seven days a week, to provide compassionate and 24-hour assistance to our patients. They vow to treat each patient with the utmost respect, show daily compassion, and meet the needs of those they serve.

Life Tastes Good at Pacific Heights Transitional Care Center

We have an incredible dietary staff that focuses on creating customized meal plans for each patient so that every individual receives the proper nutrition according to his or her medical needs.

We create an incredible dining experience by providing patients with homemade meals ranging from grilled chicken breasts, green salads tossed in balsamic dressing, and protein-packed sandwiches. Patients are also welcome to eat in the comfort of their bedrooms or in the dining room.

If patients are hungry during non-mealtime hours, a variety of beverages and healthy snacks are available.

Enjoy Life–Always

Our activities department encourages an interactive environment at Pacific Heights. According to the Harvard School of Public Health, active social interaction among seniors can lower their mortality rate and delay memory decline.

To increase social interaction, we hold monthly council meetings with our residents and their family members to plan future events and activities. Once an event or activity is selected, our activities department gathers the necessary equipment and items to carry the plan forward.

Below are some of the frequent activities we hold:

Card Games

Board Games

Arts and Crafts

Television Shows

Friday Afternoon Socials

Weekly Outings

Weekly Entertainment

Birthday Singing Grams

Holiday Parties and Festivities

*Each resident has unique limitations—some more severe than others. To combat these limitations, we break our residents into groups based on their level of ability. This ensures that all residents can interact with others and develop new hobbies without exceeding their comfort level.

*Due to COVID-19, we only hold one-on-one, social distancing activities between a staff member and a resident.
